A summary of the resource.
The development of new radiopharmaceuticals for the detection of hidden infection foci has great relevance for early detection and the selection of the correct treatment, particularly in immunosuppressed patients. In that sense, the labelling of antimicrobial peptides (AMPs) that are capable of binding specifically to the pathogenic microorganism which causes the infection, should provide a sufficiently specific agent, able to distinguish an infection from a sterile inflammation. Defensins are particularly interesting molecules with antimicrobial activity, the EcgDf1 defensin was identified from the genome of a Uruguayan native plant, Erythrina crista-galli, the ‘Ceibo’ tree. Our group has previously reported a synthetic biologically active short analogue EcgDf21 (ERFTGGHCRGFRRRCFCTKHC) successfully labelled with 99mTc. Herein we present a shorter analogue which also preserves the γ-core domain, as a pharmacophore for a potential infection detection agent. This peptide was derivatized with the bifunctional chelating agent 1,4,7-triazacyclononane-1,4,7-triacetic acid (NOTA) through a lysine linker in the amino-terminal group (NOTA-KGHCRGFRRRC) and radiolabelled with 68Ga ([68Ga]Ga-NOTA-K-EcgDf1(10)). The [68Ga]Ga-NOTA-K-EcgDf1(10) labelling procedure rendered a product with high radiochemical purity and stability in the labelling milieu. The Log P value indicated that the complex has a hydrophilic behaviour, confirmed by the biodistribution profile. The [68Ga]Ga-NOTA-K-EcgDf1(10) complex demonstrated specific binding to cultures of Candida albicans and Aspergillus niger. Its biodistribution showed renal elimination and low accumulation in the rest of the body. It was possible to successfully differentiate sterile inflammation from infection by PET images in nude mice with a target/non-target ratio of 3.3 for C. albicans and 3.7 for A. niger, respectively.

Opportunistic infections are a problem of great relevance in public health and the precise detection and localization of infection in the early stages of the dis ease is of great importance for patient management as well as cost containment. Our proposal seeks to contribute to developing a new agent that meets the needs of diagnosis and follow-up of fungal and bacterial infections, focused on the de sign of a radiotracer with the potential for recognition of hidden infection foci. Defensins are plant antimicrobial peptides that not only show activity against plant pathogens but also against human ones. A short analogue of EcgDf1 de fensin, EcgDf21d (NH2-ERFTGGHCRGFRRRCFCTKHC-COOH), was labelled through the formation of a 99mTc-HYNIC complex which was assessed for phys icochemical and biological behaviour both in vitro and in vivo. The [99mTc]Tc HYNIC-EcgDf21 labelling procedure rendered a single product with remarkably high RCP and stability in the labelling milieu. The Log p value indicated that [ 99mTc]Tc-HYNIC-EcgDf21 has a hydrophilic behaviour, confirmed by the biodis tribution profiles. The optimal uptake value was obtained for Candida albicans infection model reaching a lesion/muscle ratio of 3, this correlates with in vitro binding studies, and the lesion can be definitely observed in the scintigraphic images.

Abstract

Background: Invasive fungal diseases represent an unmet clinical need that could 29 benefit from novel immunotherapeutic approaches. Host pattern-recognition receptors 30 (e.g., Toll-like receptors, C-type lectins or Scavenger receptors) that sense conserved 31 fungal cell wall constituents may provide suitable immunotherapeutic antifungal agents. 32 Thus, we explored the therapeutic potential of the lymphocyte class I scavenger receptor 33 CD5, a non-redundant component of the antifungal host immune response that binds to 34 fungal β-glucans. 35 Methods: antifungal properties of the soluble ectodomain of human CD5 (shCD5) were 36 assessed in vivo in experimental models of systemic fungal infection induced by 37 pathogenic species (Candida albicans and Cryptococcus neoformans). In vitro 38 mechanistic studies were performed by means of fungal-spleen cell co-cultures. 39 Results: shCD5-induced survival of lethally infected mice was dose and time-dependent 40 and concomitant with reduced fungal load and increased leukocyte infiltration in 41 primary target organ. Additive effects were observed in vivo after shCD5 was combined 42 with sub-optimal doses of fluconazole. Ex vivo addition of shCD5 to fungal-spleen cell 43 co-cultures increased release of pro-inflammatory cytokines involved in antifungal 44 defence (TNF-α and IFN-γ) and reduced the number of viable C. albicans. 45 Conclusions: The results prompt further exploration of the adjunctive therapeutic 46 potential of shCD5 in severe invasive fungal diseases.

The aim of this work was to assess the influence of treatment with U-Caspofungin, on the quality of diagnostic scintigraphic images of induced lesions in nude mice undergoing both bacterial and fungal infections and to determine the level of specificity of 99mTc-tricarbonyl-Caspofungin to discriminate between fungal or bacterial infections. In vitro studies on the behaviour of the 99mTc-tricarbonyl-Caspofungin complex binding percentage at different yeast concentrations of Candida albicans or Staphyolococcus aureus were determined. The incubation was performed with and without U-Caspofungin. In vivo evaluation was performed of 6 groups of athymic mice: sham, inflammation (LPS), fungal infection with Candida albicans (CA) and bacterial infection with Staphylocuccus aureus (SA). In vitro studies showed that the uptake of the complex by both yeasts and bacteria, depends on the concentration of colony forming units (cfu), and that this uptake is favoured by the presence of UCaspofungin that increases the membrane permeability to the 99mTc-tricarbonyl-Caspofungin complex. In vivo evaluation showed low uptake in sterile inflammation model and moderate to high uptake in infections models both treated or not with U-Caspofungin. The results of biodistributions were compatible with scintigraphic images. Conclusion: The uptake of the complex depends on the load of microorganisms, but it presents high sensibility, even at low concentrations of the infecting agent. The treatment with U-Caspofungin has no influence on the quality of the scintigraphic images used for diagnosis and localization of infection foci.

<p>This study compared the possibilities and limitations of 99mTc-labeled synthetic peptides derived from two human antimicrobial peptides, namely, ubiquicidin (UBI) and lactoferrin (hLF), for the scintigraphic detection of bacterial and fungal infections in mice and rabbits. The rationale of our approach was that selected peptides accumulate in infected areas but not in sterile inflammatory lesions, because they bind preferentially to microorganisms. 99mTc-labeled human neutrophil peptides (defensins), ciprofloxacin, and human polyclonal IgG were included as control agents.</p>
<p>METHODS: 99mTc-labeled peptides and control agents were injected intravenously into animals that had been injected intramuscularly 18 h earlier with multidrug-resistant Staphylococcus aureus, Klebsiella pneumoniae, or fluconazole-resistant Candida albicans. Sterile inflammatory sites were induced by the injection of heat-killed microorganisms or lipopolysaccharide (LPS) into the thigh muscle. Up to 4 h after injection, the accumulation of 99mTc-labeled compounds in the infected/inflamed thigh muscles was determined using scintigraphic techniques and radioactivity counts in dissected tissues.</p>
<p>RESULTS: Scintigraphy revealed that 99mTc-labeled peptides UBI 29-41, UBI 18-35, UBI 31-38, hLF 1-11, and defensins, which showed preferential in vitro binding to microorganisms in a former study, accumulated at a significantly higher rate (P < 0.01) in bacterial and C. albicans infections in mice and rabbits than in inflamed tissues induced by heat-killed microorganisms or by LPS. No significant difference in the accumulation of 99mTc-labeled ciprofloxacin was observed between infected and sterile inflamed thigh muscles in mice.</p>
<p>CONCLUSION: 99mTc-labeled antimicrobial peptides UBI 29-41, UBI 18-35, UBI 31-38, hLF 1-11, and defensins accumulate significantly in tissues infected with gram-positive and gram-negative bacteria and C. albicans. Significantly lower (P < 0.01) accumulation of these peptides occurs in sterile inflamed tissues. These data indicate that the peptides preferentially tag microorganisms at the site of infection, which is in agreement with their preferential binding to the microorganisms in vitro and in vivo. 99mTc-labeled ciprofloxacin does not distinguish between infections and sterile inflammatory lesions, which implies that its specificity for the detection of bacterial infections is not warranted.</p>

The in vitro antimicrobial activity of extracts from accessions ofSolanum commersonii Dun. collected in the south of Uruguay was investigated against five microorganisms including the pathogenRalstonia solanacearum. A total of 30 extracts corresponding to organic and aqueous extracts were studied. Interestingly, most of the positive results for growth inhibition were againstR. solanacearum. The extracts were also analyzed for the presence of glycoalkaloids and lectins. Six of the ten aqueous extracts showed lectin presence and a wide variation in the type and amounts of glycoalkaloids, was found. Results indicate that there is no clear relationship between the antimicrobial activity against the five microorganisms screened and the presence or amounts of lectins and glycoalkaloids, traditionally regarded as possible antimicrobial metabolites in theSolanum genus, which indicates the presence of as yet unidentified antimicrobial compounds.
